Description of Services

 Evaluation & Management of Work Related Injuries/Illnesses

  • Provide follow-up care to injured employees who are seen in the ED and FMC.
  • Focus on complicated and difficult cases
  • Restore function and productivity
  • Return the employee back to work as soon as possible
  • Prevent or limiting impairment and disability
  • Render an impairment rating based upon objective assessment of impairment

 Independent Medical Evaluations (IMEs)

  •  Obtain a detailed medical history
  • Perform a detailed physical exam
  • Review pertinent medical records
  • Render an evidence-based opinion as it relates to impairment, work ability, and causation

  Medical Impairment Evaluations & Ratings

  •  Obtain pertinent medical history
  • Perform pertinent physical exam
  • Review pertinent medical records
  • Render an impairment rating in accordance to the appropriate AMA Guides to the Evaluation of Permanent Impairment

Fitness for Duty Medical Evaluations

  •  Obtain pertinent medical history
  • Perform pertinent physical exam
  • Review pertinent medical records and other documentation
  • Review job description and essential functions of the job
  • Render an evidence based opinion relating to the employees ability to perform the essential functions of the job with or without restrictions
  • Render an evidence based opinion whether or not the employee poses a health and safety risk

 Return to Work (Work-Ability) Medical Evaluations

  •  Perform physical exam on persons who are returning to work after being off work due to an illness or injury
  • Purpose of the evaluation is to ensure that the person can safely return to work and perform the essential functions of the job

  Second Opinion FMLA Medical Certification Evaluations

  •  Review medical certification documentation
  • Review pertinent medical records and other documentation
  • Perform medical evaluation (history and physical exam)
  • Render an opinion relating to the appropriateness of the FMLA medical certification

 OSHA Medical Evaluations

  •  Perform OSHA mandated medical exams (surveillance exams)
  • Review pertinent data (lab results, PFT results, Hearing test results, etc.)
  • Monitor the worker’s health status and ensure that it is not adversely affected by the exposure to hazards in the workplace

  Disability Exams

  •  Obtain a detail history and perform a detail physical examination for the purpose of determining if a person has an impairment which prevents them from working
  • Performed for Social Security Administration, Long Term Disability Insurance Companies, Government Agencies, Attorneys, and Private Individuals.

Employment Exams

  •  Perform post-offer/pre-placement physical examinations to determine if a person can safely perform the essential functions of the job with or without restrictions
  • Purpose of the exam is also to document any pre-existing health problems or limitations, and establish a health status baseline
